Do I need a referral to schedule an appointment?

No. We would love to see you for a free New Patient Exam to assess your orthodontic needs regardless of whether you have a DDS referral or not.

What should I bring to my first visit?
The only thing you need to bring to the New Patient Exam is the completed Health History form that is emailed to you after scheduling and yourself! Insurance information will be collected at the time of scheduling the appointment, but if there is a change before coming into the office, we ask that you bring along your new insurance information.
What can I expect during my initial New Patient Exam?
During the New Patient Exam appointment, you will meet with our treatment coordinator to discuss your orthodontic concerns, then an exam will be done by Dr. Keith. Once Dr. Keith has finished the exam, you will be presented with fees based on the severity of your treatment.
What insurance plans do you accept?

We accept all orthodontic plans. Plans that are currently “In-Network” with DK Orthodontics are:

  • Blue Cross Blue Shield
  • Delta Dental
  • Medicaid
How does billing work, and when will I receive my bill?

You will be presented with the fees the day of your New Patient Exam. You have the option of making a down payment or paying in full on that day or the day of your bonding or aligner scan.

Are payment plans or financing options available?

Yes! We know that orthodontic treatment is an investment for many families and are happy to work with you on a monthly payment plan that fits your financial needs.

Do you offer same-day or emergency appointments for broken brackets or wire pokes?

Although some days are busier than others, we always do what we can to get you seen the same day for emergency appointments. If for some reason, we are unable to accommodate the same day, an assistant will coach you over the phone on how to remain comfortable before we see you.

How do I request a copy of my medical records or imaging results?

How do I know if my insurance covers my orthopedic treatment?

We recommend doing some checking with your dental insurance provider. We will always double check before the New Patient Exam and again before your braces are put on. If there are  changes to your insurance while undergoing treatment, we kindly request that you let our office know so they can be sure to get it updated and a claim submitted to the new insurance company.
